The RSP retirement application is found on the SURS Member Website. Please complete the forms that are necessary. If you have any questions regarding the forms needed you may call SURS at 800-275-7877 and ask for the RSP department. You will need to complete the appropriate forms that apply to your specific situation. A Notary Public may be required for signatures on the application. When the forms are complete, return them to SURS along with appropriate birth verification(s) and marriage certificate, if they are not on file with SURS.
