The RSP Ad Hoc Distribution form is used by members who have retired but have not withdrawn all of their funds from the LIS. It can be used by members after their LIS activation date or by members who have declined the LIS and have chosen to take their withdrawals as needed. There are two ways to access this form. The electronic form is posted under the Quick Links on the SURS Member Website. The paper form is posted under RSP forms on the SURS website and would need to be mailed to SURS.
