If you have fulfilled the vesting requirements for retirement from your SURS-covered employer and would like to apply for retirement, log in to the SURS Member Website to access your plan’s application documents.

Please note: A complete application should contain all of the following parts:
- Retirement Application – Complete with original signature.
- Insurance Form(s) – See our Insurance page tand read about the options and learn about insurance benefits.
- Tax Form – All U.S. Citizens and Resident Aliens will default to Single Filing Status with no adjustments if the tax form is not completed and returned to SURS.
